2. When you make (or edit) a forum in the admin panel, general, manage forums, there is a field for a forum image.
You can upload that image anywhere on the server but a sensible thing to do is to probably make a folder inside phpBB/images and call it forums or similar.
Then in that field, the key part is that the location is relative to the forum folder i.e. it doesn't need the fully http etc etc. So if I had a picture called forsale.jpg in the folder layout I suggested, the field should say images/forums/forsale.jpg

Categories allow you to separate sections of the index in to related and more manageable areas.
So then you have a category with one or more forums in it. Those forums contain topics. The topics contain posts and replies (like this one).
'In Administration Control Panel (ACP) > FORUMS > Manage forums make a new forum (or edit an existing one).
Forum type: Category
Parent forum: No parent
Set permissions for category.
Then make another forum.
Forum type: Forum
Parent forum: the category you just made.
Set permissions for forum.
To set permissions, either use the option to copy permissions from an existing category or forum that has the desired permissions, or:
Administration Control Panel (ACP) > Permissions > Group forum permissions
Set the registered users to be able to see and post in it:
Registered users > Standard access
